Clear Creek Education Foundation: Student Fashion Show Application
Dear Student and Parent,
The Clear Creek Education Foundation is looking for some star power to kick‐off the sixth annual fashion show to benefit the great students and teachers of Clear Creek Independent School District. The Foundation, a 501(c)(3) organization, is searching for high school juniors, male and female, who demonstrate all that is positive about CCISD to serve as models for the Clear Creek Education Foundation annual fashion show on Sunday, May 20, 2012 at 4:30 p.m. at the Hilton NASA Clear Lake, located at 3000 NASA Parkway in Houston. This show will be produced by Lenny Matsuszewski, Jr., the nationally known fashion event producer, and will feature fashions from Dillard’s – Baybrook.
The Clear Creek Education Foundation, established in 1992, is comprised of business and community leaders; all of whom volunteer to raise funds to support the academic achievement of all students. The Foundation raises funds for several initiatives including innovative instructional grants, National Board Teacher Certification reimbursement, and funding of Clear Horizons Early College High School.
If you are a current junior in high school and are in good academic standing and involved in community activities, please submit the attached model application to your high school representative. Models will be chosen by a selection committee of business and community leaders.

Each student model’s dinner ticket will be underwritten and will be non‐transferable. Tickets for family members of models and other interested attendees are $100 per person. The Foundation anticipates an early sell out event so we encourage parents and other attendees to purchase tickets early. Tickets will not be available at the door.
Important Dates to know: Students are required to attend one of these meetings. Meeting will not last over 1 hour.
- March 27, 2012 FIRST MEETING OPTION Model Information Meeting at 6:30 p.m.
- March 29, 2012 SECOND MEETING OPTION Model Information Meeting at 6:30 p.m.
Meetings will be held at the CCISD Education Support Center Board Room. Full length and headshot photos will be taken for program and publicity. A parent is requested to attend meeting with student.
Completed applications must be mailed to the address below or returned to your high school representative no later than March 1, 2012. Models selected will be notified the week of March 14, 2012.
